New ID checks for Medicaid providers

Beginning July 1, 2025, every Nevada Medicaid provider application must include proof of the signer’s identity and signature. The signer is a natural person who signs the perjury statement. You can verify with a notary oath or by using an electronic ID system the Department uses or approves. Missing verification can delay or stop enrollment.

Signer presumed authorized; false forms are felonies

Beginning July 1, 2025, if someone signs for a Medicaid provider, the law presumes they are authorized and acting at the provider’s direction. Anyone who knowingly signs or submits a false provider application, report, or invoice commits perjury, a category D felony under NRS 193.130. Providers should set strong controls for who signs and what is filed.
