Nurse anesthetists gain flexibility with limits

Ohio replaces the prior physician-or-podiatrist direction and immediate-presence rule with collaboration under nursing law. Nurse anesthetists can provide anesthesia, epidural and spinal procedures, related assessments and tests, and order or give related treatments, drugs, and fluids. They can also direct legally authorized nurses and respiratory therapists. An actively practicing Ohio doctor, podiatrist, or dentist must request these activities verbally or in writing under facility policy. Care must follow the nurse’s training, certification, nursing board rules, and facility policies and privileges; dental collaboration stays within the dentist’s permit. The collaborator must be at the facility during the start, maintenance, and end of general anesthesia. The State Medical Board cannot bar collaboration with podiatrists practicing outside hospitals or podiatry colleges.

Patients get anesthesia notices and safeguards

Before anesthesia or an epidural or spinal anesthetic procedure, facilities must notify patients or their representatives orally and in writing. The notice must identify whether a nurse anesthetist, physician, podiatrist, or dentist will provide the care. A nurse anesthetist cannot perform a covered activity when the collaborator or facility finds it against the patient’s best interest. The patient’s medical or electronic health record must note that restriction.

Anesthesia providers must help board investigations

Nurse anesthetists must cooperate with State Medical Board or State Dental Board investigations of collaborators who requested covered care. They must also cooperate with those boards’ investigations of their own unauthorized practice of medicine or dentistry. Collaborating physicians and podiatrists must cooperate when the Ohio Board of Nursing investigates the nurse anesthetist.

Nurse anesthetists cannot prescribe take-home drugs

Nurse anesthetists cannot prescribe drugs for use outside the health care facility where they practice. This limits their ability to provide take-home prescriptions.
