The law took effect July 27, 2025. Its refill requirement applies to health plans issued or renewed on or after January 1, 2026. Plans that cover prescription hormone therapy must pay for a 12-month refill obtained at once. This applies to FDA-approved drugs that suppress, increase, or replace hormones the body does not produce at intended levels. GLP-1 drugs and GLP-1 receptor agonists are excluded. The drug must be safe to store at room temperature without refrigeration. A smaller supply applies if you request it or your prescriber says it is required. For controlled substances, plans must pay for the largest refill state and federal law allow at once. Plans may limit refills in the plan year's last quarter if a 12-month supply was already dispensed that year. During an acute dispensing shortage, prescribers may temporarily limit refills to a 90-day supply at once. They must lift that limit at the first opportunity when a sustainable supply returns. Dispensing must follow clinical guidelines that protect health while maximizing access. Plans may also use drug-use management rules consistent with these requirements.
