Rockwell Medical invented Triferic — an FDA-approved pharmaceutical drug that delivers iron to dialysis patients through the dialysate rather than via intravenous injection. The dialysis membrane, which normally removes waste products from blood into dialysate, can also work in reverse for small molecules: ferric pyrophosphate citrate (an iron complex) added to dialysate diffuses through the hollow fiber membrane into the bloodstream during normal dialysis. This allows continuous low-dose iron replacement during routine treatment sessions, eliminating the need for bolus IV iron injections. A small dialysis chemical concentrate company invented a novel drug delivery mechanism — using the dialysis membrane as a one-way drug delivery route — that changes the standard of care for anemia management in 500,000 US dialysis patients. The same Wixom, Michigan facility that mixes sodium bicarbonate and acetic acid for dialysate also manufactures a patented pharmaceutical that delivers iron through a dialysis membrane.

The US hemodialysis concentrate market has a unique structure: Fresenius, the dominant dialyzer and clinic operator, produces its own concentrate (GranuFlo, NaturaLyte) primarily for its own clinic network. DaVita (the second-largest US clinic operator with ~40% market share) competes directly with Fresenius for patients and clinic locations. If DaVita sourced its dialysis concentrate from Fresenius, it would be dependent on a direct competitor for a critical supply.
