    Polymer Dope Preparation

    Polysulfone (PSU), polyethersulfone (PES), or cellulose triacetate (CTA) resin is dissolved in N-methyl-2-pyrrolidone (NMP) solvent — typically 80 wt% NMP — with polyvinylpyrrolidone (PVP) as a hydrophilicity modifier. This spinning dope must be degassed and held at precise viscosity. NMP consumption runs ~2,000–4,000 tonnes/year globally. CTA follows a different acetylation route using wood-pulp cellulose and acetic anhydride.

  2. 02

    Hollow Fiber Spinning (Phase Inversion)

    The polymer dope is co-extruded through a precision spinneret with bore liquid (NMP/water) flowing through the inner lumen. As the nascent fiber exits into an air gap then a water bath, phase inversion causes the polymer to solidify into a porous asymmetric hollow fiber membrane. Fiber inner diameters run ~185–220 µm; wall thickness ~30–50 µm. Fresenius uses proprietary Nano Controlled Spinning (NCS) technology.

    Fiber Washing, Drying, and Conditioning

    Freshly spun fibers pass through cascade water baths to leach residual NMP solvent (regulatory limits are strict — NMP is an SVHC reproductive toxin under EU REACH). For wet dialyzers, fibers are conditioned with glycerol humectant. For dry dialyzers (Fresenius FX series), controlled drying preserves pore structure.

    Bundle Assembly and Potting

    Thousands of hollow fibers (~8,000–15,000 per dialyzer) are assembled into a bundle and inserted into a polycarbonate or ABS housing. Both ends of the bundle are potted with polyurethane resin that seals the blood-side from the dialysate-side. After curing, the potted ends are cut open to expose fiber lumens.

    Quality Testing and Integrity Checks

    Each dialyzer undergoes hydraulic permeability testing (KUF measurement), pressure hold testing for bundle integrity, and endotoxin/biocompatibility testing. ISO 8637 (hemodialysis equipment) specifies performance requirements. Ultrafiltration coefficient (KUF) and sieving coefficients for albumin and beta-2 microglobulin are measured per lot.

    Sterilization (EtO or Gamma/Electron Beam)

    Packaged dialyzers and bloodline tubing sets are sterilized by ethylene oxide (EtO) gas — the most common method for dialysis products — or by gamma irradiation. EtO sterilization requires specialized facilities with air emissions controls (EtO is a carcinogen). EtO aeration (off-gassing) takes 8–14 days at dedicated warehousing sites before shipment.

    Dialysate Concentrate Manufacturing

    Separate to dialyzer manufacturing, dialysate concentrates are formulated: acid concentrate (acetic acid, sodium chloride, calcium chloride, magnesium chloride, dextrose) and bicarbonate concentrate (sodium bicarbonate USP, sterile water). Bicarbonate concentrate must be kept dry to prevent bacterial growth. GranuFlo (Fresenius) dry acid and Naturalyte liquid acid are the two dominant US concentrate products.

    Peritoneal Dialysis Solution Filling

    For PD, sterile dextrose solution (1.5%, 2.5%, or 4.25% glucose) with electrolytes is aseptically filled into 2L or 5L multi-chamber PVC or polyolefin bags. CAPD bags use PVC with specialized plasticizers; automated cycler (APD) bags use larger-volume polyolefin films. Filling lines operate in ISO Class 5 cleanrooms. Cold-chain is not required but storage temperature must stay below 25°C.
