HUD keeps HUD-11 employee interview form

You may be asked to complete Form HUD 22019 (HUD-11 or HUD-11SP) during on-site interviews if you work as a laborer or mechanic on HUD‑assisted construction or maintenance projects. HUD is asking OMB to reinstate this collection (OMB Control No. 2501-0009); responses are voluntary and relate to enforcing Davis‑Bacon wage and reporting rules (29 CFR part 5).
