Preaward Forms Extended Three Years

The government plans to keep using Standard Forms 1403–1408 for preaward surveys and proposes to extend OMB approval for those forms for three additional years beyond the current expiration. OMB has approved the collection through October 31, 2026, and agencies will consider comments received by July 13, 2026.

The General Services Administration reports 239 respondents, 239 total annual responses, and 5,736 total burden hours annually for the preaward survey forms (SF 1403–1408). That averages to about 24 burden hours per response based on the published totals.

Single Federal Form; Surveys May Include Site Visits

All executive agencies covered by the Federal Acquisition Regulation (FAR) will use this common preaward survey form (SF 1403–1408). The necessary data for these surveys may be collected from available records or through plant visits, phone calls, and correspondence in detail commensurate with the dollar value and complexity of the procurement.
