Beginning July 1, 2026, the law sets per‑trip base rates: EMT $1,257.15; Advanced EMT $1,659.66; Paramedic $2,426.64; Paramedic‑on‑board $2,426.64. The higher paramedic‑on‑board rate applies only if a paramedic is dispatched, starts advanced life support, is told by online medical control to stay, and the providers have a payment agreement. These amounts may be adjusted by rule and this schedule ends July 1, 2027.
Beginning July 1, 2026, Utah Medicaid pays ground ambulance providers the highest rate allowed by the state assessment if CMS approves that assessment. This applies to fee-for-service ambulance claims.
From July 1, 2026 to July 1, 2027, private health plans and workers’ comp insurers must treat the state ambulance base rate and mileage rate as allowed costs when correctly charged. They must pay the ambulance provider directly, even if the provider is out of network. This applies only to claims filed under the state ambulance rate rule.
From July 1, 2026 to July 1, 2027, ambulance and paramedic providers cannot balance bill you for ambulance care. They also cannot charge above the state mileage cap or above the base rate for covered services, and they cannot bill a non‑transport fee (except in areas that include certain towns). The state sets a maximum mileage rate, and the base rate includes transport, supplies, procedures, and medication costs. Billing an uninsured person, or a service your plan denies, is not treated as balance billing. If your plan does not allow the full base plus mileage, you may be billed base + mileage − what the plan paid.
Starting July 1, 2027, the state updates ambulance base rates once each year. The new rate equals the current base rate times the medical care consumer price index. The bureau must publish the updated rates in rule and cannot change them more than once per year.
Beginning July 1, 2026, the state removes the 2027 repeal dates for three ambulance payment laws. Ground ambulance reimbursement rules for private plans and for workers’ compensation, and the state’s rate‑setting authority, stay in force past July 1, 2027. This keeps the payment framework in place.
