Form enables child annuity payments

DD Form 2828 (Certificate for Child Annuitant), OMB Control Number 0730-0011, is used to document an incapacitation that occurred before age 18 so the Defense Finance and Accounting Service (DFAS) can establish and pay a Retired Serviceman's Family Protection Plan (RSFPP) or Survivor Benefit Plan (SBP) annuity to the incapacitated individual.
