NARA seeks OMB nod to keep using facility access form

Published Date: 8/12/2026

Notice

Summary

We are proposing to request an extension from the Office of Management and Budget (OMB) of an approved information collection, Facility Access Media (FAM) Request, NA Form 6006, used by all individuals requesting recurring access to non-public areas of NARA's facilities and IT network. We invite you to comment on these proposed information collections pursuant to the Paperwork Reduction Act of 1995.

Analyzed Economic Effects

3 provisions identified: 2 benefits, 1 costs, 0 mixed.

You must file NA Form 6006 to get FAM

If you need recurring access to non-public areas of NARA facilities or its IT network, you must complete the Facility Access Media (FAM) Request (NA Form 6006). The document explicitly lists applicants such as NARA employees, contractors, volunteers, foundation employees, interns, and other non-NARA federal employees (e.g., federal agency reviewers).

Small paperwork time burden per applicant

NARA estimates the form will be completed by about 4,500 respondents annually, taking about 3 minutes per response for a total estimated burden of 225 hours per year. This is the paperwork time you would need to spend if you are an applicant.

Form required to meet HSPD-12 secure ID rules

NARA says the information collection is necessary to comply with Homeland Security Presidential Directive 12 (HSPD-12) for secure and reliable personal identification issued to employees, contractors, and others needing recurring access to government facilities and information services. The form was developed to meet that HSPD-12 requirement.
