Momnibus Act
Sponsored By: Representative Underwood, Lauren [D-IL-14]
In Committee
Summary
This bill would create a multi‑title framework to address maternal deaths and severe complications by tackling clinical care and social drivers, with a central aim to reduce preventable maternal mortality and disparities. It pairs community grants, workforce training, data and tech programs, and prison and VA reforms to improve outcomes for groups with elevated risks.

Bill Overview
Analyzed Economic Effects
14 provisions identified: 13 benefits, 1 costs, 0 mixed.
Longer WIC help after birth
If enacted, the bill would extend WIC postpartum eligibility from 6 months to 24 months after birth. It would also extend breastfeeding coverage from 1 year to 24 months. The Agriculture Secretary would report to Congress within 2 years on effects such as breastfeeding rates and disparities.
Bigger maternal data and research push
If enacted, the bill would fund and expand national maternal health data, surveillance, and research. It authorizes large appropriations for CDC programs, PRAMS, ERASE MM, and an NIH IMPROVE research initiative ($73.4 million per year for FY2027–2032). The bill requires reviews, data modernization, an American Indian/Alaska Native study, and expanded maternal mortality review topics and reporting.
Medicaid demos and telehealth rules
If enacted, States could apply to test new perinatal payment models under Medicaid and CHIP from FY2027 through FY2031. The bill would also allow Medicaid demonstrations, starting one year after enactment, to use telehealth tools for screening and monitoring pregnancy complications for up to one year after pregnancy.
More local help for pregnant families
If enacted, HHS would run large competitive grants to fund community groups that help pregnant and postpartum families. Grants could pay for housing, transport, nutrition, job support, group prenatal care, and other social‑determinant services. The bill would fund community engagement grants so maternal mortality review committees include diverse community members and reserve funds for Tribal and Urban Indian organizations.
More maternal mental health help
If enacted, HHS would set up the MOMS MATTER grant program to integrate maternal mental health and substance use services into maternity and primary care. The bill would also fund workforce grants ($15 million per year, FY2027–FY2031) for training, scholarships, and capacity building to grow maternal mental health providers.
New programs for incarcerated pregnant people
If enacted, the Department of Justice would create model maternal care programs in at least six federal prisons and fund state and local programs for pregnant and postpartum incarcerated people. Grants and BOP programs would fund nutrition, perinatal workers, trauma‑informed mental health care, reentry help, and require independent oversight and outcome reporting.
Protecting moms from climate risks
If enacted, HHS would create a Climate Health Equity grant program to protect pregnant people, those within one year postpartum, and young children in high‑risk counties. Grants (to up to 10 recipients) could pay for air conditioners, weatherization, filtration, evacuation or housing help, and related supports. The bill also requires a federal climate risk Strategy for maternal health, training grants for health schools, and an NIH consortium on birth and climate research.
Respectful care, training, and rules
If enacted, HHS would fund training grants ($5 million per year, FY2027–FY2031) and grants to hospitals to create respectful maternity care compliance programs. The bill would define "respectful maternity care" and who counts as maternity care providers and perinatal health workers. HHS and the National Academies must issue guidance and studies and the GAO would report on access and compliance.
Federal task forces and emergency plans
If enacted, HHS would form a cross‑agency Task Force to coordinate efforts to reduce preventable maternal deaths and disparities. The bill also requires a Task Force to make recommendations on respectful, equitable maternity care during public health emergencies and directs CDC public education and public emergency data reporting for pregnant and postpartum people.
25% Byrne grant cut for noncompliant states
If enacted, beginning six months after enactment and each year after, a State without a law like the federal anti-shackling rule would have its Byrne justice assistance grant reduced by 25 percent for that fiscal year. Money withheld from noncompliant States would be reallocated to States that meet the law's requirements.
More digital tools and monitoring
If enacted, HHS would award limited grants to expand telehealth, remote patient monitoring, and provider digital tools in high‑need areas. The bill would also fund a study on how maternity technologies can produce bias and recommend best practices to protect privacy and equity. Grants are small and time‑limited.
More maternal vaccination funding
If enacted, the bill would add pregnant and postpartum individuals as an explicit target of maternal vaccination programs. It would expand settings named for outreach and increase authorized funding to $17 million each year for fiscal years 2027 through 2031. The bill would also add raising vaccination rates among pregnant and postpartum people, including those from racial and ethnic minority groups, to the program's activities.
VA maternity reporting and funding
If enacted, the VA would publish a report within one year and annually through FY2031 on VA maternity care and maternal outcomes for veterans. The bill also authorizes $15 million per year for FY2027–FY2031 to support VA maternity coordination.
Grants to grow perinatal workforce
If enacted, HHS would fund grants to expand and diversify the perinatal workforce. The bill would authorize $15 million each year for fiscal years 2027 through 2031 for nursing scholarships and $15 million each year for perinatal education program grants. Grants would pay scholarships, expand training capacity for nurse practitioners, midwives, physician assistants, and perinatal health workers, and favor programs that recruit diverse students and place trainees in shortage or high-disparity areas. Recipients would report annually on students, demographics, and placements.
