Military spouses must file DD Form 3145-4

If you are a military spouse seeking DoD jobs at GS-15 and below (or equivalent), you must complete the Military Spouse Priority Placement Program Self-Certification Checklist (DD Form 3145-4) to get priority consideration for competitive or excepted service positions at DoD activities in the U.S. and U.S. territories. You must provide evidence of appointment eligibility and proof of marriage to an active-duty service member (including a copy of permanent-change-of-station orders).

Collection used to certify placement preference

DoD gaining activities will use the completed DD Form 3145-4 to certify a military spouse's preference eligibility for possible appointment into vacancies at DoD activities in the U.S. and U.S. territories. The form collects evidence of appointment eligibility and marriage to a current active-duty member.

Time burden for each form submission

Each response to DD Form 3145-4 takes an average of 30 minutes. On average respondents file 4 responses per year, so the typical military spouse spends about 2 hours per year; the collection estimates 69,362 respondents, 277,448 annual responses, and 138,724 annual burden hours in total.
