Medical ID marks with card fee limits

Beginning October 1, 2026, Alabama offers an optional, discreet medical-condition mark on driver licenses and nondriver ID cards. It covers physical or mental conditions that may make communication with first responders harder. These include communication impediments, autism spectrum disorder, traumatic brain injury, epilepsy, schizophrenia, and cognitive disabilities. You must provide proof of diagnosis from a physician licensed in Alabama. The Alabama Law Enforcement Agency cannot charge just for the mark. Beyond existing fees, the agency may charge card production and issuance costs plus no more than $0.10; qualifying students under the law's student exemption pay no fees under this section. Revenue collected under this section must support the Driver Communication and Safety Enhancement Program, with excess money returning to the State General Fund each fiscal year.
