Coverage protections for out-of-network primary care referrals
For referrals made beginning July 1, 2026, Iowa health insurers cannot deny covered care solely because an out-of-network primary care provider referred you. Deductibles, coinsurance, and copays cannot exceed those for referrals from an in-network health professional. Insurers may request proof of a signed direct primary care agreement, through a written statement or an agreement copy. This agreement provides primary care services for a set period for a service charge. Eligible primary care providers include doctors in family or general practice, pediatrics, internal medicine, obstetrics, or gynecology; advanced registered nurse practitioners; and physician assistants. Iowa’s Insurance Commissioner can issue rules to carry out these protections.
