Beginning July 1, 2026, Iowa’s massage board can inspect locations that advertise or offer massage services. Civil fines reach $1,000 per offense for unlicensed practice, employing unlicensed therapists, or operating without a required establishment license. The establishment-license requirement begins January 1, 2027. Each continuing day after an order or citation counts separately, with a $10,000 maximum for a continuing violation. The board can suspend or revoke licenses or impose probation conditions. Establishment revocation grounds include public complaints, misconduct or disqualifying acts at the business, and fraudulent applications. Pending criminal charges against an owner or officer can trigger emergency suspension; businesses cannot operate while suspended. Beginning July 1, 2026, licensed people—or those claiming licensure—must show documents when an officer investigating a complaint of illegal services requests them. Required documents are a valid Iowa therapist license, government-issued photo ID, and an establishment license when applicable. Failure to comply remains a serious misdemeanor.
Beginning January 1, 2027, covered Iowa massage businesses must get a separate establishment license and pay a fee. The law sets no dollar amount; the Department sets license terms and renewal periods by rule. Exclusions cover customer-site visits, stand-alone self-operated devices, qualifying health care settings, approved student clinics, and approved massage schools. Sole practitioners are also excluded when they alone offer massage at a location under their exclusive physical control. Owners must submit fingerprints for a national criminal history check, with possible exemptions for previously submitted fingerprints. The board can deny licenses based on applicants’ or controlling persons’ criminal histories or lack of good license standing. Grounds include human trafficking, sexual exploitation of minors, sexual abuse, money laundering, child abuse, and dependent adult abuse. Businesses must meet safety, sanitation, building, fire, and health-code standards, adding consumer protections and business duties. The board can inspect before licensing, every two years, or after complaints. Businesses must keep records for three years from creation, including therapist names and license numbers, client appointment calendars or receipts identifying the therapist, and other required records. Board rules govern applications, inspections, records, license transfers, and operations.
