The law expands Medicaid for employed people with disabilities. If you are under 65 and your family income is under 250% of the poverty guideline, the state can ignore your earnings and unearned income when checking eligibility. It also ignores up to $10,000 in resources for an individual or $21,000 for a couple. Money in retirement or medical savings accounts, and other accounts the department approves, can also be ignored.
The law requires most Iowa Health and Wellness Plan members to work at least 80 hours each month to keep coverage. It does not start until the state gets federal approval, builds a real-time reporting system, and adopts rules. You are exempt if you are under 19, age 65 or older, SSA‑disabled, medically frail, caring for a child under 6, in a high‑risk pregnancy, receiving unemployment, or in substance use treatment (up to six straight months). The director may also grant good‑cause exemptions. If federal rules later ban work requirements, the department must end the plan if allowed; if not, the state may move to a different approved plan after new state law approval.
The law creates a DHHS information technology fund to pay for system upgrades and modernization. Unused money stays available through the next fiscal year and earns interest for the fund. Any unspent money in the Public Assistance Modernization Fund on June 30, 2025 moves to the new fund, and the old fund is repealed.
The health department works with federal agencies to line up work and training rules across public assistance programs, including SNAP. This can change who must work or train to get benefits once new rules are set.
