Beginning August 1, 2025, Louisiana Medicaid health plans cannot deny, delay, reject, or take back claims solely because other coverage exists unless all required coverage details appear in the state’s Medicaid Eligibility Verification System. Required details include insurer name, address and phone; policyholder name, policy and group numbers; coverage start date; and coverage limits. Louisiana must put known information about other legally responsible insurers or payers into that system. Plans must report missing coverage within two business days, including required coverage details and any extra information the state requires. The state must update the system within three business days after receiving notice. When plans receive payment from another responsible payer, they must notify the provider in writing or electronically within five business days. That notice must include the explanation of benefits, payer contacts, policyholder details, coverage start date and coverage limits. Louisiana must withhold at least $25,000 per violation, with a minimum of $100,000 for multiple violations or a pattern.
Louisiana Medicaid health plans cannot change a provider’s submitted claim or pay it at a lower service level. The health secretary can authorize exceptions through a statewide rule, but both legislative health and welfare committees must approve it. Required reviews and audits after payment, and actions based on them, remain allowed. Louisiana must withhold at least $25,000 from a health plan for each violation.
