No-cost PrEP options in private plans
Health plans in Maine must cover HIV prevention drugs. Plans must cover at least one FDA‑approved drug for each way it is given. If the drug has a USPSTF A or B rating and meets federal guidance, you pay $0. Plans may not require prior authorization or step therapy for the covered options. If your plan used to require approval for your drug, it must now cover it with no out‑of‑pocket cost. Pharmacists can bill plans directly and be listed as the provider; plans cannot add extra training or collaboration rules beyond state law.
Get PrEP from your pharmacist now
You can get PrEP from a pharmacist. They can give 30 to 60 days of oral PrEP or one injectable dose. You must have a negative HIV test in the past 7 days, or the pharmacist orders and verifies a test. You cannot have signs of acute HIV, and the pharmacist must check for unsafe drug interactions and provide CDC‑based counseling. The pharmacist must notify your primary care doctor or give you a list if you do not have one. Without a separate prescription, the pharmacist cannot give more than 60 days of oral PrEP or another injection within two years.
MaineCare pays pharmacists for PrEP
By January 1, 2027, MaineCare pays pharmacists for prescribing, dispensing, and giving HIV prevention drugs. Pharmacists can bill MaineCare directly and be listed as the provider on the claim. The department creates a payment model to increase access and applies to federal CMS for any needed Medicaid plan changes by January 1, 2027. The department cannot add training or collaboration rules beyond state law.
Funds to set up MaineCare PrEP access
The law funds setup costs to run these MaineCare changes. For fiscal 2026–27, the total is $192,824: $44,264 from the General Fund, $146,951 Federal, and $1,609 Other Special Revenue. This pays for one‑time technology work and startup for pharmacist payments.
