Medicaid may cover licensed doulas

Beginning January 1, 2026, Montana Medicaid may cover services from licensed doulas. Coverage depends on department rules and federal Medicaid law. If the service is added, enrollees can pay less out of pocket for doula support. This authority ends December 31, 2030.

New state license for doulas

Starting January 1, 2027, doulas must hold a Montana license to practice as licensed or state‑certified doulas. To get licensed, applicants pay fees, file a complete application, meet department-set competencies, and have no unprofessional conduct or active discipline. The department may issue limited licenses for supervised experience. People in other fields and volunteers may give supportive services if they do not use the “doula” title. If you worked as a doula before January 1, 2026, you can qualify by showing equivalent experience to the department by December 31, 2027. The department sets rules and treats doula licenses like other professional licenses. This program ends December 31, 2030.
