New York health plans with major medical coverage must ask about organ donor registration during sign-up, renewal, and first online portal setup. Every form must include space to join or decline the Donate Life Registry for organs, eyes, and tissues. The form must show this exact message: “You must fill out the following section: Would you like to be added to the Donate Life Registry? Check box for 'yes' or 'skip this question'.” Some NY State of Health marketplace forms are excluded as specified in law.
