Bill Overview

Analyzed Economic Effects

2 provisions identified: 1 benefits, 0 costs, 1 mixed.

Patients protected from extra ambulance bills

For covered ground ambulance care, patients cannot face extra bills after paying their plan’s in-network cost share. This includes emergency care at the scene or during transport, plus medically necessary transfers between health care facilities. The rules apply to health plans issued, renewed, or extended on or after the law’s effective date. Self-funded group plans must opt in by notifying Oregon’s Department of Consumer and Business Services. Public Employees’ Benefit Board and Oregon Educators Benefit Board coverage is excluded unless those boards opt in. The department can impose civil penalties, with a $1,000 cap for repeated violations.

Health plans face ambulance payment rules

Participating health plans must pay ground ambulance providers the established local rate for covered services. Without a local rate, plans must pay at least 325% of Medicare’s rate. The local rate comes from a public cost review where the service began, through local government operations or contracts. Providers must submit local rates annually and within five calendar days of changes; the state must publish a public database. Oregon’s Department of Consumer and Business Services must set rules to carry out these requirements. By September 15, 2026, it must report to interim health committees on billing complaints before and after implementation. The report also covers contracts signed on or after January 1, 2026, and premium effects after that date. The reporting requirement ends January 2, 2027—not the billing protections. The law caps implementation spending at $502,113 for the Division of Financial Regulation in 2025–27, from fees and other revenues, excluding lottery and federal funds.
