For Oregon plans issued, renewed, or extended on or after January 1, 2026, breast reconstruction coverage must include listed surgeries using your own tissue, including combinations with implants. A licensed provider must prescribe surgery under accepted care standards; coverage includes medically needed hospital and outpatient care, procedures, imaging, and revisions. Coverage and care review rules, including prior approval, cannot be less favorable than for other covered breast reconstruction. Plans must have enough providers across their service area to avoid unreasonable delays or arrange out-of-network care. When networks are inadequate, plans cannot impose higher deductibles, out-of-pocket limits, copays, or coinsurance than in-network care, and must pay outside providers at least their average in-network rate for comparable services.
