Beginning May 13, 2025, the Greenville Health System board has 12 to 17 members. The Greenville County Legislative Delegation appoints all members. For each house-district seat, House members from the listed districts, with any Senator for those areas, must nominate one person; the delegation accepts or rejects and repeats until the seat is filled. Six seats are tied to residence in these House districts: 18 or 20; 5, 17, or 19; 16, 21, or 35; 22 or 24; 23 or 25; and 7, 10, 27, or 28. Appointees must be qualified electors, and those six must live in the named Greenville County district at appointment and through their term. The rest are at-large: at least four must live in Greenville County; two must live in the City of Greenville; no more than three may live in the same city; and City of Greenville appointees go to the city for review.
