New Tennessee license path for foreign-trained doctors

Beginning January 31, 2027, eligible doctors trained outside the United States can get a provisional Tennessee medical license. It lasts two years, with a possible one-year extension, and requires a nonrefundable $300 application fee. Applicants need an acceptable medical diploma, a permanent ECFMG certificate for foreign medical graduates, and the required licensing exam. They need at least three years of approved postgraduate training abroad or one year of postgraduate training in the United States. They must show good moral character, good professional standing and no pending disciplinary actions within the past five years. Applicants must be U.S. citizens, qualified aliens under federal law, or have applied to live or work in the United States. They also need a Tennessee clinical job offer from a board-identified employer. Approved workplaces include federally qualified health centers, rural health clinics, community health centers, accredited residency-program employers, and other board-approved supervising-doctor employers. Practice is limited to eligible employers, with board notice and approval required for an employer change. Supervising doctors must be onsite, hold an active, unrestricted Tennessee license, and practice in and be board-certified in the same specialty. After two years of satisfactory active Tennessee practice following provisional licensure, the board must grant a full, unrestricted license to qualifying doctors. They must remain in good standing, show medical skill, and pass United States Medical Licensing Examination Steps 1, 2, and 3. Rulemaking authority takes effect upon enactment; the medical boards can set employer and supervision rules and are encouraged to collect and share outcome data.
