S2903 — Safe Step Act
Creates a mandatory, transparent exceptions process for medication step therapy so patients and prescribers can quickly request coverage of a non-preferred drug when medically needed. The bill sets timelines, standard forms, and public notice rules to make exception requests faster and clearer. - Patients and prescribers would get a clear path to request exceptions, submit clinical rationale on a standard paper or electronic form, and designate a third-party advocate. Plans must respond within 72 hours for routine requests and within 24 hours for emergencies, and granted exceptions must remain covered for at least one year. - Group health plans and insurers would have to publish the process, forms, required supporting information, and contact details on plan materials and websites. The bill limits information requests to what is strictly necessary and stops pharmacy benefit managers or third-party administrators from withholding data needed for reporting. - Plans would report counts and outcomes of exception requests by circumstance, specialty, and condition starting not later than three years after enactment and each year thereafter by October 1. The Secretary of Labor would compile a Congressional summary of trends, and the Secretary must issue final implementing regulations within six months with the rule applying to plan years that begin at least six months after enactment.
S1552 — Living Donor Protection Act of 2025
Prohibits insurance discrimination against living organ donors and expands workplace leave to cover donation surgery and recovery. It also directs federal health agencies to update public education on living donation. - Stops life, disability, and long-term care insurers from denying, cancelling, refusing to issue, or changing coverage or premiums solely because someone is a living organ or bone marrow donor when no unique actuarial risk exists. Enforcement is handled by State insurance regulators. - Adds organ donation, preparation for donation, and recovery to the Family and Medical Leave Act and to federal civil service leave rules. Federal employees who use part of the 12-week FMLA leave to donate may substitute other available leave where possible. - Requires the Department of Health and Human Services, in coordination with the Department of Labor as appropriate, to review and update educational materials and can issue public service announcements and web updates within six months to explain the changes and resources.
S2287 — Palliative Care and Hospice Education and Training Act
This bill would expand the palliative and hospice workforce. It would create coordinated federal programs for education, training, career incentives, public information, and NIH research to strengthen palliative and hospice care across hospitals, clinics, homes, and long-term care settings. - Families and patients: Would fund public information and targeted materials for Medicare, Medicaid, and Veterans Health Administration beneficiaries and medically underserved groups, and promote palliative care starting at diagnosis across pediatric and adult populations. - Health professionals and workforce: Would fund education grants, physician training, faculty career awards, fellowships, and career incentive awards across medicine, nursing, social work, chaplaincy, pharmacy, psychology, and related fields. Fellowship awards would be capped at up to $150,000 per person and the fellowship program would be limited to funding no more than 24 programs. Career incentives would carry service obligations of at least five years. - Research and coordination: Would require the National Institutes of Health to develop a nationwide palliative care research strategy and, beginning January 1, 2026, permit NIH to conduct or support palliative care research. The bill also authorizes a coordinating hospice and palliative nursing training program. Would authorize about $15 million per year for the workforce programs and $5 million per year for hospice and palliative nursing training for fiscal years 2026–2030.
S2709 — Telehealth Modernization Act
Would extend and expand Medicare telehealth flexibilities to keep audio-only visits, broaden originating sites, and bring more virtual prevention and home-based care into Medicare. It also clarifies clinic payment for 2026–2027, adds oversight tools, and requires telehealth claim modifiers for certain hospice encounters. - Families and beneficiaries: Would keep audio-only telehealth through September 30, 2027 and let more originating sites receive telehealth. It would allow virtual Diabetes Prevention Program (MDPP) services online for 2026–2030 and let beneficiaries receive MDPP across state lines for payment. - Providers and clinics: Federally Qualified Health Centers and Rural Health Clinics would have a payment rule for telehealth services in fiscal years 2026 and 2027 and could treat telehealth costs as allowable under their payment methods. Hospices would need a telehealth modifier on face-to-face encounters before recertification starting January 1, 2026. - Medicare program and care models: Would extend the Acute Hospital Care at Home waiver through 2030 with required studies and reports, add a Master List and prepayment reviews for certain durable medical equipment starting January 1, 2028, and require an Inspector General fraud analysis for lab tests in 2026.
S1241 — Sanctioning Russia Act of 2025
Harsh, automatic sanctions and trade penalties would be triggered if Russia refuses to negotiate with Ukraine, violates a peace deal, invades again, or seeks to subvert Ukraine's government. The bill would require visa and property-blocking sanctions, target major Russian banks, ban U.S. energy exports to Russia, restrict U.S. investments and listings tied to Russia, and force duties of at least 500% on Russian imports.
S410 — Love Lives On Act of 2025
Keeps survivor benefits after remarriage. The Love Lives On Act of 2025 would stop remarriage from automatically ending key payments and health coverage for spouses of deceased service members and veterans. It would do this by changing provisions in title 38 and title 10 of the U.S. Code to protect Dependency and Indemnity Compensation, special pension, Survivor Benefit Plan annuities, and TRICARE dependent status. - Surviving spouses: Veterans' surviving spouses would remain eligible for Dependency and Indemnity Compensation and special pension even if they remarry. This preserves ongoing monthly payments that remarriage would otherwise have ended. - Survivor Benefit Plan (SBP): The Department of Defense could not terminate an SBP annuity solely because a surviving spouse remarries. For spouses who remarried before age 55 and before enactment, the bill sets transitional rules that require annuity payments to resume either monthly starting one year after enactment or immediately for those who elected a specific child-transfer option as of Dec. 31, 2019. The bill also adds statutory language clarifying treatment of survivors of members who die on active duty. - TRICARE dependents: TRICARE's definition of dependent would expand to include a remarried widow or widower whose later marriage ended by death, divorce, or annulment.
