HRSA Revises Health Center Forms

If you run or work for a Health Center Program award recipient or look-alike, HRSA is updating the forms used by nearly 1,400 health centers and more than 16,000 sites that serve over 32 million people. The updates apply to forms used to apply for grants and non-grant opportunities, renew designations, report progress, and request changes to scope of project; the current forms will expire April 30, 2026.

HRSA Adds and Removes Specific Forms

HRSA will add new forms such as a Grant Number form, a Checklist for Adding a Transitional Care in a Carceral Setting Site to Scope, and QIF Transitions in Care for Justice-Involved Populations progress and evaluative report forms. HRSA will remove forms including the Applicant Qualification Criteria Form, Environmental Information and Documentation, Form 3A: Look-Alike Budget Information, and Form 4: Community Characteristics to streamline information collection and reduce burden.
