Starting July 1, 2026, Hawaii health plans must recognize licensed pharmacists who contract with the plan. Plans must cover pharmacist services within their licensed scope to the same extent they cover those services from other providers. The plan must pay or reimburse the pharmacist or pharmacy, but the law does not set a payment amount.
Hawaii Medicaid includes pharmacists who provide medical care as individual practitioners in its fee-schedule payment system. Medicaid coverage and payment changes take effect only after federal approval, which the Department of Human Services must seek. The state appropriation act sets the fee schedule using a percentage of the 2000 Medicare fee schedule, and the Legislature must specify later adjustments. Payments cannot exceed federal limits, the current-year Medicare fee schedule, state appropriation limits, or the provider’s billed amount.
