Anesthesia coverage for public employees

State, county, city, and school employee plans must include the required anesthesia coverage. This applies when those governments self-insure or run the plan. Beginning 2026-01-01, these plans cover medically necessary anesthesia for covered procedures. The Departments of Insurance and Central Management Services share enforcement.

Health plans must cover anesthesia

Beginning 2026-01-01, group and individual health plans and managed care plans cover medically necessary anesthesia for any covered procedure. Plans cannot deny payment just because anesthesia lasts longer than a preset time. The rule applies when your policy is amended, delivered, issued, or renewed on or after 2026-01-01.

HMOs and limited plans add anesthesia

Beginning 2026-01-01, if you are in an HMO, it must follow the new anesthesia coverage rules. Limited health plans follow these rules only when anesthesia is part of their limited covered services. Some exclusions for small employers and certain Medicare supplement policies still apply.
