The health department prepares and posts a plain summary of Illinois advance directive law. The summary includes the state POLST form. It also posts Spanish versions of the Living Will, the Health Care Power of Attorney, the Mental Health Treatment declaration, the summary, and the POLST form. These materials may be online only.
The health department develops and publishes a single, uniform POLST form that meets national rules. It can be created, signed, and revoked electronically using secure, technology‑neutral systems that meet digital signature or medical record standards. This does not replace a doctor's power to issue a do‑not‑resuscitate order. The department must publish the updated form reflecting this law by January 1, 2015.
The health department studies whether Illinois should create a statewide registry for advance directives and POLST forms. The registry would let residents submit forms and let providers find them quickly when giving care. The study is done with many stakeholder groups. The department must file the study with the General Assembly by January 1, 2021.
The health department approves and posts POLST training and offers easy, yearly training. Training is available to staff and contractors at licensed intermediate and skilled nursing facilities. It is also available to other health care providers who use the POLST form. The goal is informed choices and accurate forms.
