Medicaid post-mastectomy coverage and pay floor

Medicaid covers post-mastectomy care. The Department adopts rules based on the state’s model to run this benefit. On and after January 1, 2016, payments to hospitals and providers for this care cannot be below Medicare rates. The Department may reduce some other Medicaid rates under existing law starting July 1, 2012.

More insurance protections for HMO and limited plans

HMOs, limited health service groups, and voluntary health service plans must follow many listed Insurance Code rules. This can expand required benefits and consumer protections for people in these plans. For limited health service groups, the law does not force coverage beyond their limited service scope.

Post-mastectomy care for public workers

State, county, and city self-insured employee health plans must cover post-mastectomy care. These plans must also follow the listed Insurance Code coverage rules. For the State Employees Group Insurance Program, coverage applies to members and covered dependents. The Department of Insurance enforces the county and city rules, and the state assigns enforcement between the Insurance Department and Central Management Services for state employee coverage.

Artery disease screening covered for at-risk

Starting January 1, 2027, new or renewed health plans must cover a peripheral artery disease screening test when medically needed. This applies if you are at risk under American College of Cardiology and American Heart Association guidelines.
