Temporary Medicare fees for long-term care pharmacies

For plan years 2026–2027, Medicare drug plans would owe long‑term care pharmacies a supply fee for each specified prescription filled at the Medicare maximum fair price. The fee would be $30 in 2026 and would rise in 2027 by the annual percentage set in law. Plans would have to pay this at the same time as other reimbursements and could not reduce those other payments. Plans that do not pay would face a civil penalty of at least $10,000 per missed fee. To offset costs, the government would repay plans the total fees paid within 18 months after each plan year, on top of other Part D payments.
