Louisiana patients skip approval for qualifying generics

Louisiana commercial insurers cannot require advance approval to dispense or reimburse qualifying non-opioid generic drugs. The drug must be chemically equivalent to a brand-name drug and FDA-approved as interchangeable. Its wholesale acquisition cost must be $250 or less per prescription; this is not the patient's out-of-pocket price. A board-certified physician must prescribe it, or an authorized provider whose specialty certification covers the condition being treated. The physician must be licensed in Louisiana and certified by an American Board of Medical Specialties member board or a state-recognized equivalent. The rule covers commercial plans with prescription-drug coverage, not Medicaid or other government plans. It applies to new policies, contracts, and health plans issued beginning January 1, 2027. Existing plans must comply at renewal, no later than January 1, 2028.
