Louisiana Medicaid adds covered doula care

Louisiana Medicaid plans with maternity benefits must cover doula support before, during, and after birth—not medical or midwifery care. Members must have a licensed healthcare provider certify their pregnancy or postpartum status and must request or agree to services. Coverage includes at least five prenatal visits and three postpartum visits, each lasting up to 90 minutes. It also includes labor and birth support, including cesarean births, breastfeeding help, parenting education, and support after pregnancy loss. The law covers inpatient care and, when appropriate, outpatient care for a medically accepted reason. That care must occur in a state-registered facility and come from a properly registered professional under applicable guidelines or certifications. The Louisiana Department of Health sets coverage details and payment rates using market benchmarks and rates in similar states. Doulas need Louisiana Doula Registry Board approval for managed-care payment; all Medicaid plans must accept approved doulas under Medicaid rules. Doulas remain free to practice without reimbursement, and discriminatory terms in Medicaid plans or contracts are void. New managed-care plans issued on or after August 1, 2025, must comply. Earlier plans must comply at renewal, no later than January 1, 2026.
