Louisiana workers' compensation petitions must name the parties and give their addresses. They must describe the injury's time, place, type, and cause, or give equivalent information identifying the parties. Petitions must give facts about the specific unpaid, missing, or disputed benefit and briefly state the benefits or other relief sought. Employers and payors can file disputed claims against employees, dependents, or beneficiaries over benefits or any other workers' compensation issue. This authority is no longer limited to fraud claims or appeals of medical-director decisions.
