Medicaid cap and possible service changes

From April 1, 2025 to March 31, 2026, state Medicaid spending is capped at $33,417,285,000. The Budget Director can adjust the cap for FMAP changes, provider revenues, local payments, minimum‑wage costs, and disasters. If projections show the cap will be passed, the Medicaid director must roll out a savings plan that can change reimbursement or covered benefits, consistent with federal law. The budget director and health commissioner check spending every quarter by service and region, and DOH posts quarterly reports online. DOH must post the savings plan and send it to key lawmakers at least 30 days before it starts, unless a public health emergency requires faster action.

More money for unemployment benefits

Starting April 1, 2025, $165,000,000 goes to the Unemployment Insurance Benefit Fund. The money pays state and federally authorized unemployment benefits during the fiscal year. If you collect unemployment, this funding helps keep your checks coming.

Keeping Medicaid payments and wages on track

The law lets Medicaid funds cover minimum‑wage‑related raises and fringe benefits for workers in Medicaid‑funded programs. DOH can use funds for pharmacy benefit management, PASRR checks, legal help on federal issues, and insurance data matching. The state may pay CMS for drug costs for people enrolled in both Medicare and Medicaid who use Medicare Part D. The Comptroller can hold local district shares in an interest‑bearing account to pay providers on time. The Budget Director can move health dollars among accounts with required filings.

Community mental health funding and rules

Starting April 1, 2025, $94,917,000 funds adult community mental health services, with reimbursements and advances tied to local program years. Family care funds can pay a substitute caretaker for up to 14 days and up to $774 a year for a client’s personal needs, based on financial need. OMH may keep existing contracts without new bids to maintain services, with Budget Director approval. Through July 1, 2026, the OMH commissioner alone chooses who may apply to run comprehensive psychiatric emergency programs. Directors who are federal representative payees in licensed facilities may use resident funds to pay for the resident’s care, under federal rules.

Funds for federal nutrition benefits

From April 1, 2025, $3,210,000 in federal USDA funds pays nutrition benefits and program costs run by the state. If you use SNAP or WIC, these dollars help fund your benefits and providers.

Small boost for homeless veterans housing

Beginning April 1, 2025, $42,000 supports housing‑related services for homeless veterans. The money comes from the Homeless Veterans Assistance Fund and pays program services and expenses.

State pays its Social Security share

Starting April 1, 2025, $20,650,000 covers state employer fringe costs. $20,300,000 pays the state’s Social Security contribution. $350,000 pays the metropolitan commuter transportation mobility tax for affected state employees. These payments fund employer‑side costs, not extra take‑home pay.

Three-day bridge funding and spending approvals

The law provides two one‑time payments totaling $20,000,000 to pay state non‑personal service bills from April 1–3, 2025. No money in this act can be spent until the Budget Director issues a certificate and files it with the Comptroller and key lawmakers. The Legislature and Judiciary are exempt. These steps keep government bills paid during the brief transition and control when spending starts.
