New York birth certificates for foreign adoptees

You can get a New York birth certificate if you were adopted abroad, at any age. The adoption must be finalized or recognized and reported to the state health commissioner. No other U.S. birth record can exist for you, and no New York certificate of birth data can exist. To file, give proof the adoptive parent lived in New York at the time, or proof the adoption was lawfully done in New York. If the adoption was abroad, include the foreign adoption papers and a certified translation. Also give evidence of your date and place of birth, and proof of IR-3, IR-4, IH-3, or a successor visa. The local registrar sends your paperwork to the commissioner, who issues a new certificate listing your name, sex, date and time of birth, place of birth, your mother’s maiden name, and your father’s name.
