Hospitals must explain stillbirth and fetal-death certificates

Beginning November 1, 2026, Oklahoma hospitals must act when a fetal death happens or a miscarriage is treated. Before discharge, the hospital gives the patient or her designee a paper or digital copy of a State Health Department form. A staff member also tells the patient or family that they can request the listed certificates and how to do it. The Health Department posts the form on its public website. It explains how to request a fetal death certificate or a certificate of birth resulting in stillbirth, and lists Vital Records contact information.
