    Pharmaceutical-Grade Water Production (WFI)

    Source water is purified to Water for Injection (WFI) standards via multi-effect distillation or validated membrane systems (reverse osmosis + electrodeionization + ultrafiltration). WFI must be pyrogen-free, endotoxin-free, and meet USP/EP specifications. Dedicated stainless steel loop systems maintain water at 70–80°C continuously to prevent microbial growth. This is the most capital-intensive upstream step and a bottleneck for new plant qualification.

  2. 02

    Active Ingredient Compounding

    Pharmaceutical-grade sodium chloride (NaCl, 99.9%+ purity, ultra-low endotoxin), dextrose monohydrate, sodium lactate, potassium chloride, and/or calcium chloride are weighed and dissolved in WFI in stainless steel compounding vessels. Formulations are verified analytically for pH, osmolarity, and clarity (e.g., 0.9% NaCl for normal saline, 5% dextrose for D5W).

  3. 03

    Filtration and Bioburden Reduction

    The compounded bulk solution passes through 0.22-micron sterilizing-grade membrane filters to remove particulates and microorganisms. Integrity testing of filters is performed before and after each batch. Pre-filtration bioburden reduction steps (0.45-micron pre-filters) extend filter service life. Filtered solution is held in sterilized surge tanks before filling.

    Container Manufacturing or Procurement

    IV bags are manufactured via Blow-Fill-Seal (BFS) technology or from pre-made film from qualified converters. Traditional PVC bags require DEHP plasticizer; modern polyolefin (PP/PE multilayer) and EVA bags are DEHP-free. California AB 2300 (signed September 2024) bans DEHP in IV bags statewide by 2030, effectively forcing nationwide conversion to polyolefin or EVA containers.

  5. 05

    Aseptic Filling

    In ISO Class 5 (Grade A) cleanroom environments, sterilized solution is filled into bags or BFS containers using automated high-speed filling lines (10,000–30,000 units per day per line). Fill volume accuracy, seal integrity, and container closure systems are monitored in real time via automated vision systems.

  6. 06

    Terminal Sterilization (Autoclaving)

    Filled and sealed bags undergo terminal sterilization in large industrial autoclaves at 121°C steam under pressure, achieving a sterility assurance level of 10⁻⁶. This step is not applicable to BFS products (intrinsically sterile) or heat-labile formulations requiring aseptic-only processing.

  7. 07

    Inspection, Labeling, and Packaging

    Each unit undergoes 100% automated visual inspection for particulates, container defects, and fill volume. Units pass through label application (product name, lot number, expiry, barcode) and are secondary-packaged into polyethylene overwrap for moisture and light protection. Cartons are palletized and stored at controlled temperature (15–30°C).

  8. 08

    Release Testing and Distribution

    Finished product is quarantined until QC release: sterility testing (21-day compendial), endotoxin (LAL test), pH, osmolarity, assay, and container integrity tests. Release takes 2–4 weeks. Approved lots ship via temperature-controlled truck to hospital distribution centers and GPO warehouses, with just-in-time delivery standard across the industry.
