  1. 01

    API Synthesis / Procurement

    Active pharmaceutical ingredient is synthesized or purchased. Most ophthalmic APIs (fluoroquinolones, prostaglandins, cyclosporine A) are synthesized in India or China via multi-step organic chemistry. Highly potent APIs require controlled synthesis with chiral chemistry or fermentation-derived precursors.

  2. 02

    API Release & Excipient Preparation

    API is released after CoA testing (HPLC purity, particle size, endotoxin). Excipients — HPMC/CMC viscosity agents, sodium hyaluronate, BAK preservative, tonicity agents, pH buffers — are weighed and dissolved in Water for Injection (WFI). All water systems must meet USP WFI specifications per USP <1231>.

  3. 03

    Sterile Compounding / Bulk Formulation

    API is dissolved in excipient matrix under cleanroom conditions (ISO 5–8). For cyclosporine emulsions, high-shear homogenization is required. pH, osmolality, and clarity are validated before sterile filtration. Isotonicity (~290 mOsm) and near-neutral pH (6.5–7.5) are required.

    Sterilization — 0.22-μm Filtration

    Bulk solution is passed through a 0.22-μm sterilizing-grade membrane filter (PVDF/PTFE/PES from Merck Millipore, Pall, or Sartorius). Filter integrity is validated pre- and post-filtration via bubble point test. Most ophthalmics are aseptically filled due to heat sensitivity of APIs.

    Aseptic Filling — Primary Container

    Multi-dose bottles: sterile bulk filled into LDPE dropper bottles under ISO 5 RABS/isolator. Unit-dose: blow-fill-seal (BFS) machines form LDPE, fill, and seal in a single step. Injectables: filled into Type I borosilicate glass vials, stoppered with coated elastomer stoppers, then crimp-sealed.

    Container Closure Integrity & Sterility Testing

    Each lot undergoes container closure integrity testing, sterility testing per USP <71>, bacterial endotoxin per USP <85>, particulate matter per USP <789>, and extractables/leachables screening. Ophthalmic sterility requirements are among the most stringent in pharma.

    Labeling & Secondary Packaging

    Filled units are labeled, serialized (DSCSA track-and-trace), inserted into cartons with patient leaflets. Cold-chain packaging applies to cyclosporine emulsions, prostaglandin analogues (latanoprost: 2–8°C before dispensing), and ophthalmic biologics.

    QC Release & Distribution

    QC release batch record reviewed; certificate of analysis issued. Product shipped to wholesalers (AmerisourceBergen, McKesson, Cardinal) or hospital pharmacies. FDA drug shortage reporting obligations apply during distribution disruption. Generic ophthalmics flow through GPO contracts that exert severe price compression on manufacturers.
