TennCare authorizes one new PACE program as a pilot in a grand division that had no PACE in 2024. If you are being considered for nursing home placement, managed care plans and counselors must consider PACE and have referral protocols. This adds a community‑based care option for frail, elderly TennCare members when appropriate.
If a PACE program is in a county with 366,200–366,300 people (2020 or later census), it may expand only into neighboring counties with populations of 12,700–12,800, 32,800–32,875, or 108,600–108,700. This can extend PACE access to residents of those qualifying contiguous counties.
TennCare ensures PACE programs meet CMS and state quality standards. PACE organizations must send regular data and reports to TennCare and CMS. Beginning January 1, 2026, TennCare sends a PACE report to the House and Senate leaders and finance chairs every January 1. The TennCare director can issue rules under the state administrative procedures to carry this out.
To seek the single authorized pilot, a provider must first send a letter of intent. The letter must name the applicant, list proposed zip codes with a service area map, and give the proposed PACE center site. TennCare checks that applicants meet federal PACE rules at 42 CFR Part 460 and certifies they are qualified in the proposed area. Applicants must submit a market analysis that estimates eligible people, explains methods and assumptions, shows expected market capture and saturation, and proves unmet need.
