Beginning July 1, 2026, TennCare does not pay for care meant to help someone live as a different gender or to treat distress about that. This includes surgeries, hormones, and puberty blockers. The rule applies to care done in Tennessee and to care you receive while you are in Tennessee, including telehealth from out-of-state providers. The law defines sex by anatomy and genetics at birth, which sets how these limits apply.
Starting July 1, 2026, TennCare can still pay for care that treats a congenital defect, early puberty, disease (not including gender dysphoria), or a physical injury. TennCare may also keep paying for gender identity‑related care that began before July 1, 2026, but only until March 31, 2027. Your treating doctor must write and file a certification, with findings, that stopping the care would be harmful. The grace period covers the same ongoing care and does not allow new or different procedures for gender identity.
