Lasting vaccine exemptions and classroom access for kids

Beginning May 7, 2025, qualifying vaccine exemption forms stay valid throughout enrollment in Utah public education and remain in permanent records after transfers. Medical exemptions require a completed exemption form and a licensed health care provider’s signed notice that vaccination endangers the student’s life or health. Personal exemptions require a completed form stating personal or religious beliefs. Public school agencies offering both remote and in-person learning cannot deny exempt students the in-person option based on vaccination status. State and local health departments keep their legal powers to contain infectious disease.

School vaccine records follow kids without fees

Beginning May 7, 2025, schools must request immunization records at enrollment and keep them in students’ permanent records. Schools cannot charge for receiving or reviewing immunization records or exemption forms. Within five business days after enrollment, a designated official must check receipt, review vaccine compliance, and identify missing information. Missing or deficient records require conditional enrollment, with the required notice sent within five days after that placement. When a student transfers, the school must send the immunization record upon the legally responsible adult’s request. If not sent then, it must accompany any later transfer of the student’s records to the new school.
