    Air compression and pre-treatment

    Atmospheric air is drawn in by large compressors and filtered to remove particulates, moisture, CO2, and hydrocarbons. For cryogenic ASU plants this is the first stage; for PSA systems compression drives the adsorption cycle. Feed air quality is critical — oil, moisture or CO2 contamination degrades zeolite sieve performance and cryogenic column efficiency.

    Cryogenic distillation OR Pressure Swing Adsorption separation

    Two fundamentally different technologies operate in parallel markets. Cryogenic Air Separation Units (ASU): air is cooled to −183 °C, liquefied, then fractionally distilled — oxygen (boiling point −183 °C) separates from nitrogen (−196 °C) and argon (−186 °C), yielding 99.5%+ purity O2 in bulk. PSA/VPSA systems: compressed air passes through zeolite molecular sieve beds (typically lithium-exchanged 13X zeolite) which preferentially adsorb nitrogen; oxygen-rich gas (90–96% purity) exits the vessel. PSA systems cycle between adsorption and desorption every few seconds using two towers in alternation. WHO accepts ≥82% purity for most medical uses; cryogenic grade (99.5%+) is required for some surgical and neonatal applications.

    Liquid oxygen storage and tanker distribution

    Cryogenic-grade liquid oxygen (LOX) produced at ASU plants is stored in vacuum-insulated double-walled cryogenic tanks. Bulk LOX is transported by specialized cryogenic tanker trucks (ISO tanks) — the rate-limiting logistics layer. India's 2021 crisis was caused not by insufficient production but by a fleet of only ~1,200 cryogenic tankers, far too few to redistribute supply to surge locations in real time. Chart Industries is the dominant US manufacturer of cryogenic transport trailers, having acquired the Worthington Industries cryogenic trailer business.

    Cylinder filling or on-site generation

    For cylinder supply: gaseous oxygen is compressed into steel or aluminum cylinders (Type 1) or composite-wrapped cylinders (Type 3) at filling stations. Medical cylinders must meet pharmaceutical-grade standards (FDA NDA, EU Ph. Eur. monograph, IP 2010). Luxfer Holdings and Catalina Cylinders dominate the aluminum medical cylinder market; Luxfer reports >15 million cylinders in active use worldwide. For on-site PSA: the generator itself is the end production point, delivering 90–96% O2 directly to the hospital pipeline.

    Hospital pipeline and manifold distribution

    Bulk LOX tanks or cylinder manifolds feed hospital medical gas pipeline systems (MGPS). Copper or stainless steel pipelines distribute oxygen at low pressure (~4 bar) through ward outlets, ICU headwalls, operating theatre booms, and emergency department drop-down ports. Pipeline integrity (leak testing, pressure monitoring) is a regulated activity under HTM 02-01 (UK) and NFPA 99 (US). Leakage is a persistent issue — WHO estimates hospitals can lose 20–40% of oxygen to leaks in poorly maintained systems.

    Flow regulation and patient delivery

    Oxygen is delivered to patients through flow-regulating valve and regulator assemblies at each outlet. Delivery modes range from low-flow nasal cannula (1–6 L/min) to Venturi masks (24–60%) to High-Flow Nasal Cannula systems (HFNC, up to 60 L/min — consuming 2–4× the oxygen of a standard mask). COVID-19 drove explosive adoption of HFNC; during the 2021 US delta wave, just 22 HFNC patients consumed 41% of one hospital's total oxygen supply. Pulse oximetry (SpO2 monitoring) determines when oxygen is needed; lack of oximeters is a critical gap in LMICs.
